RESUMO

INTRODUCTION: Ellis-van Creveld (EVC) syndrome is an autosomal recessive disorder predominantly characterized by a disproportionate dwarfism, ectodermal dysplasia, postaxial polydactyly, and congenital heart malformations and pulmonary hypoplasia. OBJECTIVE: In this article, we hereby present a case of a 6-year-old Brazilian boy with EVC syndrome who presented a rare oral lesion as well as a remarkable number of classical and uncommon oral and dental features. CASE REPORT: Clinical and radiographic examination revealed multiple enamel hypoplasia, teeth agenesis, conical teeth, lower canine rotation, bilateral posterior crossbite, taurodontism of deciduous and permanent molars and delayed tooth eruption, dental caries, and absent vestibular sulcus. Additionally, a whitish lobulated nodule located in the alveolar ridge in the anterior region of the mandible was noted. Anatomopathological examination was compatible with the diagnosis of peripheral odontogenic fibroma (POF). In a 10-month clinical follow-up, no signs of recurrence were observed. CONCLUSION: In view of the characteristic oral findings of EVC syndrome and the potential for recurrence of POF, the pediatric dentist plays an essential role in clinical follow-up, planning and preventive, and rehabilitative treatment.

Intrusive dislocation is the most frequent type of traumatic injury in the primary dentition, characterized by displacement of the tooth into the alveolus. Intrusions require a thorough physical examination, radiographs of diagnostic value, and follow-up of the case, since the prognosis is unfavorable, mainly due to the probability of damage to the permanent tooth germ. This work aims to report a clinical case of intrusive dislocation in the primary dentition, from diagnosis to follow-up. This is a male patient, 3 years old, who attended in the Emergency of the Children's Specialty Clinic State University of Londrina (UEL) due to dento-alveolar trauma in the antero-superior region that occurred three days ago. On clinical examination, tooth 61 had grade II intrusion (one-third of the crown visible), and tooth 62 with grade I intrusion, with more than one-third of the crown visible. Radiographically, a buccal inclination was observed, with no fracture of the bone plate. Thus, we opted for conservative treatment, waiting for the spontaneous re-eruption of the teeth involved. Furthermore, parents were informed about the possible consequences of this type of trauma and the need for clinical and radiographic follow-up consultations. After eight months of follow-up, the teeth erupted satisfactorily. It is extremely important to establish a protocol for observation and control of the traumatized tooth, observing signs and symptoms of the tissues involved in the trauma to reduce sequelae in both dentitions

Introduction: Oral ranulas are pseudocysts resulting from the retention of mucus or extravasation of saliva that often affect children. This article aims to report two clinical cases of oral ranulas and discuss the treatment using the micromarsupialization technique. Case report 1: 6-year-old girl, attended the Emergency Room of the Baby Clinic of the State University of Londrina complaining of a "ball under the tongue" lasting approximately 3 months. Clinically, reddish bubble of approximately 1.5 cm in diameter was noted located on the right side of the dome-shaped mouth floor, sessile base and fluctuating. Case report 2: 5-year-old girl, attended the same service with a complaint of "bubble on the tongue" lasting approximately 2 months. Clinically, pink-colored bubble similar to the mucosa of approximately 1 cm in diameter was observed located on the left side of the floor of the mouth with a circumscribed shape, sessile base and fluctuating. Based on the clinical aspects and the current history of the lesions, the diagnosis was oral ranula and the surgical treatment performed was micromarsupialization. In a clinical follow-up of 8 and 6 months, respectively, total regression of the lesions was observed, with no clinical signs of recurrence. Conclusion: Based on the reported clinical cases, it can be concluded that the use of micromarsupialization technique represents an effective surgical procedure in children due to its quick execution, conservative approach, without the use of local infiltrative anesthesia, well tolerated and with good prognosis.

RESUMO

The aim of this study was to evaluate and compare the clinical and radiographic success rates of pulpotomies in primary molars using a new stain-free tricalcium silicate cement (Bio-C Pulpo) versus a conventional white MTA (MTA Angelus) as a pulpotomy medicament for primary teeth. Seventy primary teeth in 33 patients were randomly divided into two groups, MTA Angelus (n = 34) and Bio-C Pulpo (n = 36). Clinical and radiographic follow-up examinations were conducted at seven days (except radiographic follow-up), and at one, three, six and 12 months. At 12 months, the clinical success rate of the MTA Angelus group was 100 percent (28 out of 28), and the radiographic success rate was 100 percent (28 out of 28). In comparison, the clinical success rate for the Bio-C Pulpo group was 100 percent (29 out of 29), and the radiographic success rate was 100 percent (29 out of 29). No significant differences were found between the groups at any follow-up evaluation. According to Fisher´s exact test, a significant difference was observed between the pulpotomy agent and tooth discoloration (p < 0.001). The success rates were 100% for the Bio-C Pulpo group, with no case of discoloration, and 10.31% for the MTA Angelus group. Both MTA Angelus and Bio-C Pulp pulpotomy medicaments are appropriate options for pulpotomies in primary teeth, and enable high clinical and radiographic success rates. However, more long-term studies are required to test the new Bio-C Pulpo medicament.

RESUMO

INTRODUCTION: The aim of these studies was to evaluate the effects of regular volleyball training on salivary SIgA and alpha-amylase in female children and adolescents. MATERIAL AND METHOD: In the cross-sectional study, 115 female children (12.6 ± 2.2 years) participating in a Volleyball training program were classified as beginners, intermediate, and competitive level. The children were evaluated regarding caries index, body mass index (BMI), cardiorespiratory fitness, and countermovement jump. In the longitudinal study: 54 girls (intermediate and competitive groups) trained for 8 weeks and were re-evaluated at weeks 4 and 8. RESULTS: The SIgA secretion rate and alpha-amylase activity did not present correlations with training category, age, caries index, or training status. A weak positive correlation was detected between IgA secretion rate and BMI (r = 0.20, p < 0.05). After 8 weeks of training, VO2max (p < 0.005) and jump height (p < 0.005) improved in the competitive girls. No differences were observed in salivary secretion rate, SIgA concentration and secretion rate, or alpha-amylase activity at weeks 4 and 8. CONCLUSIÓN: We concluded that despite improving physical fitness, 8-weeks of recreational volleyball training are not able to improve salivary SIgA secretion or alter alpha-amylase activity in female children and adolescents

RESUMO

AIM: This study aimed to evaluate the effect of a laser-activated bleaching agent containing titanium dioxide (TiO2) nanoparticles on enamel roughness and hardness. MATERIALS AND METHODS: Twenty human premolars were randomized into two groups according to the bleaching treatments performed: HP - 35% hydrogen peroxide and HP + TiO2 - 30% hydrogen peroxide containing TiO2 light-activated by diode laser (980 nm). It was performed two bleaching sessions with an interval of 7 days. Microhardness and roughness of the enamel were assessed at three times: T0 - Before 1st appointment, T1 - after 2nd appointment, and T2-7 days after 2nd appointment. RESULTS: The HP + TiO2 did not cause changes on enamel roughness and hardness and presented the same effects of the HP. CONCLUSIONS: Both bleaching agents showed no difference between them. Then, it is possible to conclude that both are viable for clinical use during in-office dental bleaching technique regarding the microstructural changes that they might cause.

RESUMO

OBJECTIVE: To investigate the effect of two light-curing protocols on mechanical behavior of three bulk-fill resin composites (BFRC) considering their optical properties. METHODS: One increment of 4 mm thickness of the bulk-fill resin composites Opus Bulk Fill, Tetric N-Ceram and Filtek Bulk Fill Flow were submitted to two different light-curing protocols: Sp - irradiance of 1000 mW/cm2 (20 s); Xp - irradiance of 3200 mW/cm2 (6 s). To assess the influence on the mechanical behavior it was studied polymerization shrinkage by X-ray microtomography (n = 3), Vickers hardness (n = 10) at the top and bottom surfaces of the samples, irradiance reaching the bottom surface (n = 3) and absorbance spectrum during the light-curing time interval (n = 3). Data were analyzed by two-way ANOVA test for parametric data and Kruskal Wallis test, followed by Wilcoxon or Mann-Whitney U post-test, for non-parametric data. RESULTS: All BFRCs contracted when light-cured, with greater contraction for Xp. Filltek Bulk Fill Flow showed highest polymerization shrinkage, for both Sp and Xp. All BFRCs showed minor hardness values on the bottom surface, with greater reduction for Xp. All BFRCs exhibited a decrease in irradiance at 4 mm depth. A decrease in absorbance intensity throughout the light-cure was observed, except for Opus Bulk Fill. CONCLUSIONS: Regardless BFRCs composition, the light-curing protocol with lower irradiance and longer exposure time results in lower polymerization shrinkage and higher hardness. The higher irradiance in a shorter time interval compromises the mechanical behavior of the resin composites, which may result in undesirable clinical outcomes.

RESUMO

PURPOSE: We aimed to evaluate whether the Facial Injury Severity Scale (FISS), proposed by Bagheri et al in 2006, can predict intervention needs in the operating room (OR), length of hospital stay, and need for support by other specialties. PATIENTS AND METHODS: Data were collected from the medical records of trauma patients from a public tertiary hospital between January 2009 and December 2015, related to age, gender, comorbidities, habits, history of maxillofacial trauma, etiology, and presence and location of fractures and lacerations, in addition to type of treatment performed and period of hospitalization. A score was applied to each patient according to the FISS. Adjusted multinomial logistic regression models were generated, and all analyses were performed with the SPSS statistical package (version 17.0; IBM, Armonk, NY). RESULTS: The final sample consisted of 469 medical records. The mean age was 31.38 ± 14.13 years. Traffic accidents were the most frequent cause of trauma (41.2%), followed by interpersonal violence (29.4%). The most commonly fractured bone was the mandible (32.9%), and the mandibular angle was the most affected region (29.0%). Reports of alcohol intake resulted in a 100% increase in the need for treatment of fractures in the OR. Patients with FISS scores greater than 5 presented with 18 times the chance of needing intervention in the OR and a greater possibility of hospitalization longer than 3 days (P < .01). The mean length of hospital stay was 8.14 ± 6.02 days, with a statistically significantly longer hospitalization period for smokers (P < .0001). Patients with any type of comorbidity were more likely to require support from other specialties (P = .022), and those with FISS scores greater than 5 were 6.6 times more likely to need this support (P < .0001). CONCLUSIONS: Higher FISS scores may predispose patients to a longer hospitalization time, a greater chance of undergoing surgical procedures, and a greater need for follow-up by other medical specialties.

RESUMO

Background: Academic stress may impair mucosal immunity and expose dental students to an increased risk of infections. Objective: to assess stress scores in dental students and their relationship with variation in SIgA levels. Methods: All students (n = 289) were invited to take part of the study, and 207 (71.63%) effectively participated, being 152 (73.4%) females. At the day of data collection, the students answered The Dental Environmental Stress Questionnaire (DES) and unstimulated saliva samples were collected for determination of salivary flow rate and SIgA concentration and secretion rate. Results: Mean DES scores were higher in females (78.97 ± 16.42), but no correlations between the sum of DES scores and salivary parameters were observed (P=0.08). A moderate inverse relationship was observed between SIgA secretion rates and the subscales Academic Performance (P=0.01), Interpersonal relationships (P=0.02) and Difficulties and Insecurities about Professional Future (P=0.05). A weak correlation was found between SIgA concentration and the items Amount of assigned classwork (P=0.02), Lack of confidence in self to be a successful dentist (P=0.01), Lack of time for relaxation (P=0.01), Financial responsibilities (P=0.02) and Personal physical health (P=0.005). Weak correlations between SIgA secretion rates and DES items were also found for Lack of cooperation by patient in their home care (P=0.003), Patients being late or not showing up for their appointments (P=0.02), Lack of self confidence to be a successful dentist (P=0.008), Personal physical health (P=0.019), and others. Conclusion: Different sources of stress were observed among first to fifth year students and some of these stressors may negatively impact on salivary SIgA secretion. (AU)

(AU)

RESUMO

This research aims to probe the porosity profile and polymerization shrinkage of two different dual cure resin cements with different dentin bonding systems. The self-adhesive resin cement RelyX U200 (named RU) and the conventional Allcem Core (named AC) were analyzed by x-ray microtomography (µCT) and Scanning Electron Microscopy (SEM). Each cement was divided into two groups (n = 5): dual-cured (RUD and ACD) and self-cured (RUC and ACC). µCT demonstrated that the method of polymerization does not influence the porosity profile but the polymerization shrinkage. Fewer concentration of pores was observed for the conventional resin cement (AC), independently the method used for curing the sample. In addition, SEM showed that AC has more uniform surface and smaller particle size. The method of polymerization influenced the polymerization shrinkage, since no contraction for both RUC and ACC was observed, in contrast with results from dual-cured samples. For RUD and ACD the polymerization shrinkage was greater in the lower third of the sample and minor in the upper third. This mechanical behavior is attributed to the polymerization toward the light. µCT showed to be a reliable technique to probe porosity and contraction due to polymerization of dental cements.

RESUMO

BACKGROUND/AIM: Dental avulsion damages periodontal tissues and may induce dentoalveolar ankylosis and root resorption after replantation. The aim of this work was to evaluate the effects of light-emitting diode (LED) therapy after tooth replantation. MATERIAL AND METHODS: The upper right incisors of 36 male Wistar rats were extracted and replanted after 15 min. The control group (Co, n = 18) was submitted to replantation and the LED group (n = 18) was submitted to replantation and LED therapy (940 nm, 4 J cm-2 ) for three consecutive days (0, 24, and 48 h). Six animals from each group were euthanized at 7, 14, and 21 days for histological analysis. The upper left incisors were used as a negative control. Data were compared using the Fisher exact test, considering P < 0.05. RESULTS: After 7 days, the Co (73.3%) and LED (72.2%) groups presented areas of periodontal ligament necrosis. However, periodontal ligament necrosis was more frequent in the LED group at 14 (70%, P < 0.05) and 21 days (43.2%, P < 0.05) in relation to the Co group (33 and 21.6%). Inflammatory cell infiltration was more frequent in the Co group at 7 days (100%) than the LED group (24.4%, P < 0.05), but increased in the LED group after 21 days (83.3%, P < 0.05). Bone necrosis was more frequent in the LED group after 14 (79%, P < 0.05) and 21 days (60%, P < 0.005) than in the Co group (45% and 20%). Bone repair was evident in the Co group at 14 days. All of the replanted teeth had pulp necrosis. CONCLUSION: The results suggest that the application of LED therapy up to 48 h after tooth replantation may delay periodontal ligament repair.

RESUMO

Objective: To perform cross-cultural adaptation of the Dental Environment Stress (DES), to test its construct validity and reliability, and to identify the sources of stress among Brazilian dental students. Material and Methods: The DES was transculturally adapted to Portuguese using translation/back-translation, review by an expert bilingual committee and consensus building. The first version was tested in a sample of 42 dental students to check the understanding level of the alternatives. The final version was applied to all the students enrolled in a Brazilian Public Dental School. Construct validity was assessed through factor analysis, performed by principal components analysis and Varimax rotation and reliability by internal Cronbach's alpha coefficient (95% CI). Wilcoxon rank sum was conducted to test for gender and Kruskall-Wallis for year comparison. Multivariate analysis relied on ordinal logistic regression modeling. Results: Factor analysis revealed five factors that possessed eigenvalues greater than 1.5 and together explained 46.88% of the total variance. Internal consistency of each factor was adequate, with Cronbach's alpha ranging from 0.65 to 0.84. 'Examinations and grades' (82.80%) was the highest scored item. Females presented higher rates, as well as second's and fifth's years students. The entering students were generally concerned with factors related to "Academic Performance", whereas clinical year students with "Insecurity Concerning their Professional Future". Conclusion: The Portuguese version of the DES presented good results, thus it could be a valid instrument to assess the factors of perceived stress in Lusophone countries, subsidizing the development of strategies to minimize the stress and optimize school performance.
